You may not need all of these documents . Many will not be available at the start of the appeal process but may be obtainable within the timeframe allocated for the appeal

If parents wish to commission independent expert reports, they are advised to book these in good time . Many experts are booked up months in advance

Refusal to assess or refusal to issue a statement

1.      Letter refusing to carry out a statutory assessment or to issue a statement

2.      Appeal form ; ( this can be downloaded from www.sendist.gov.uk or http://www.justice.gov.uk/guidance/courts-and-tribunals/tribunals/send/index.htm;)

3.      Annual Review Reports for the last 3 years

4.      School reports / Home tuition reports for the past 3 years

5.      Any other therapists reports for the last 3 years; e.g.

a.      Speech and Language Therapists

b.      Occupational Therapists

c.      Educational Psychologists

d.      Clinical Psychologists

6.      Medical Reports for the last 3 years

7.      Social Services reports/ Core Assessments  for the last 3 years

8.      Common Assessment Framework ( CAF ) – if applicable

9.      Any exclusion reports or incident reports from school or  respite settings

10.   Correspondence with the LA ( letters and emails )

If you do not have any of this information we can obtain it on your behalf by writing to the school for a copy of their records or to the LA or relevant professionals.

Appeal against the Contents of a Statement or Cease to Maintain a Statement

1.      Final Statement

2.      Letter Accompanying Final Statement or “ Cease to Maintain “ letter

3.      Appeal form ;  ( this can be downloaded from www.sendist.gov.uk or http://www.justice.gov.uk/guidance/courts-and-tribunals/tribunals/send/index.htm; )

4.      Full set of appendices ( listed on the  Statement )

5.      Annual Review Reports for the last 3 years

6.      School reports / Home tuition reports for the past 3 years

7.      Any other therapists reports for the last 3 years; e.g.

a.      Speech and Language Therapists

c.      Educational Psychologists

d.      Clinical Psychologists

8.      Medical Reports for the last 3 years

9.      Social Services reports/ Core Assessments  for the last 3 years

13.     Details ( prospectus and Ofsted ) of the parents preferred school and the LA preferred school and     any assessment reports from the schools
